What To Bring For Your First Appointment
Being well prepared for your appointment will ensure that the doctor has all of the pertinent information to provide the best possible care for you and your family. We ask that you arrive 15 minutes prior to your reserved appointment to allow enough time to fill out all necessary medical documents and to take care of any of your personal needs. In order to complete all patient forms, you will need to bring the following items:
✓ Your Driver License or Photo ID
✓ Your Insurance Card (if applicable)
✓ A list of all your medications and past surgeries
✓ Any X-rays or Dental Records from your previous dental office should
✓ be sent to our office 2 weeks prior to your first visit with us

Forest Crossing Family Dental is an insurance-friendly practice that is delighted to help you maximize your dental benefits, regardless of which insurance company our patients use. We have made every effort to accept most of the common dental insurance plans offered.
Payment & Financing
Forest Crossing Family Dental strives to make your dental experience as painless as possible, which includes paying for services rendered. For this reason we have worked hard to provide a variety of investment options to help you receive the dental care you need and deserve.
Broken Appointments Policy
We pride ourselves in reserving the necessary time to provide quality dental care. A specific amount of time is reserved exclusively for your appointment. Last minute cancellations or not showing up for a reserved appointment seriously disrupts patient care, increases cost and deprives others from needed medical/dental assistance. If you must change your appointment, we require at least 48 hours notice to avoid a cancellation fee being charged to your account. This fee will be determined based upon the reserved time of the operatory and administrative cost.
What To Bring For Your First Appointment
Your first visit to Forest Crossing Family Dental establishes a vital foundation for our relationship with you. From the moment you walk through our door, our number one goal is to assist you in feeling welcomed, comfortable and in control of your dental experience. We’ll take the time to get to know each other, hear your concerns, answer your questions, and review your medical history.
Your first appointment will be with one of our Doctors and our Dental Hygienist. It will consist of a thorough Comprehensive Exam and Oral Cancer Screening. We require a Panoramic X-ray to rule out the presence of Oral Cancer, Cysts, Impacted Teeth and other serious abnormalities of the lower face. For proper patient care it dental cleanings are performed only on patients without major gum disease or oral infections.
Following your examination, the doctor will take time to review your medical and dental history, as well as the results of your thorough exam. He will then develop a continuing Personalized Care and/or Treatment Plan to help you achieve your optimal oral health. For more involved cases, we’ll invite you back for a complimentary consultation and treatment plan review.
As dental professionals, we pride ourselves in taking the time to educate our patients and to assist them in coming to informed decisions regarding their own course of treatment. We recognize that everyone adopts change (even change for the improvement of one’s own health) at their own pace. As such, we’ll partner with you to formulate a treatment plan that is both emotionally and financially comfortable for you.
